Your first visit to Little Harpeth Children's Dentistry involves a few special steps so that we can get to know you. To understand what to expect, please read through this page. Please note ALL children seen must be FULLY vaccinated AND a birthparent must accompany them to the intial visit.

We recommend you save some time by printing out and completing the patient forms in advance of your appointment. Failure to do so will result in a delay in your scheduled appointment time and an "in-office paperwork fee."

Please print and fill out these forms so we can expedite your first visit. It is REQUIRED that New Patient Paperwork is brought in completed to your initial appointment. Failure to do so on your part will result in a NPP fee when done in office and the inability to see you at your scheduled time.
